Doctors usually prefer surgery to treat soft tissue sarcomas. Surgery might be able to remove cancer but, it highly relies on the site and size of a sarcoma. The goal of the surgery is not just to remove the entire tumour but, also the normal tissues present around it, around 1-2 cm. The reason behind this is to make sure that no cancer cells are left behind. After removal, the tissue will be overlooked under the microscope and then the doctor will check if the cancer is growing in the margins of the specimen or not.
If the doctors figured out that the cancer cells are found at the edges of the removed tissues, then it can be termed as positive margins. In other words, it also makes sense that cancer cells might have been left behind. If any of the cancer cells are left behind after surgery then further treatment methods such as radiation or other surgery might be needed.
If the doctors could not figure out that the cancer cells are found at the edges of the removed tissues, then it can be termed as negative or clear margins. If sarcoma is removed with clear margins, then the chances of its returning diminish. In this situation, surgery may be the only treatment needed. If the tumor is present in the abdomen region then it can be hard to remove it as the tumor could be next to vital organs that can’t be taken out.
Amputation and limb-sparing surgery –
in the past, amputation was an option available with the surgeons. It is a procedure in which the arms and legs were treated by removing the limb. These days it is barely used. In place of that standard surgery is used to remove the tumor without amputation. In other terms, it is known as limb-sparing surgery. In order to replace the removed tissue, a tissue graft or an implant may be used. Then if necessary, radiation therapy is being followed.
In some cases, amputation can’t be avoided. It might be the only way possible to remove cancer completely. Along with cancer, it might be necessary to remove a few bones, muscles, blood vessels, and key nerves. Amputation may be the best option if removing the tissues would mean leaving a limb that doesn’t work well or would result in chronic pain.
Surgery if sarcoma has spread –
If the sarcoma has spread to a few particular sites such as lungs or other organs, then if possible all of cancer will be removed. The removal process includes the original tumor site along with the areas of spread. If the doctors find out that it is impossible to remove all of the sarcomas, then they will not head towards the Sarcoma Surgery in Dubai.
Most of the time, when the sarcoma spreads over then surgery alone cannot be the cure. But, if the sarcoma has been spread to some particular spots in the lungs, then the metastatic tumors can be removed. This type of surgery can cure patients or at least assist them with long-term survival.
Lymph node dissection –
There is an even chance that if the lymph nodes near the tumor are enlarged then cancer may be found over that portion. During the surgery, a few of the swollen nodes may be sent to the lab and checked for cancer. The area around the lymph nodes will be removed if cancer is found. After surgery, if necessary radiation might be used in that area.
Treatments used with surgery –
Before surgery, if necessary both chemotherapy and radiation may be given. The term assigned to this process is called neoadjuvant treatment. For complete removal, it is used to shrink the tumor. In order to treat high-grade sarcomas, radiation or chemo can be given before surgery. This is when there is a high risk of cancer spreading. If chemo or radiation is used after surgery then it is called adjuvant treatment. This process mainly aims to eliminate any cancer cells that may be left in the body. This will lower the risk of cancer that may come back.
